The intake questionnaire takes about 5-10 minutes to fill out. Upon submitting the form you will get a text, call or email from the HD Genetics team, Wes Solem or BJ Viau. These calls allow us to gather a bit more information and then we’ll connect you with one of our regional clinical trial educators based on your location.
